Example meeting details
Keep meeting details factual and short. The form asks for Review date, Chairperson, Attendees, Apologies and Review period. A small business might complete it like this:
- Field Example entry Review date15 September 2026 Chairperson Managing Director Attendees Operations Manager, Quality Coordinator, Service Manager Apologies Finance Manager Review period January to August 2026
Do not overcomplicate this section. The important point is that the record clearly shows the period being reviewed and that appropriate people took part. If someone with responsibility for a key action cannot attend, record the apology and assign the action owner clearly in the outputs table.
How to write useful review input notes
Each required input has space for Evidence / discussion and Decision / action. This is where many SMEs either write too little or turn the minutes into a transcript. A better approach is to write one or two evidence-based sentences, then a specific decision.
Previous actions
For previous actions, record whether each item is closed, delayed or still open. Useful wording is specific:
| Prompt | Example entry |
|---|---|
| Evidence / discussion | Three actions were raised at the last review. Two are closed, with evidence saved in the corrective action register. One supplier review action remains open because the supplier scorecard was not updated in August. |
| Decision / action | The Operations Manager will complete the supplier scorecard update by 30 September and attach the updated record to the action evidence. |
This shows follow-up, ownership and evidence. It also connects the minutes to other QMS records instead of leaving decisions isolated.
Changes in context and interested parties
This input is about whether anything material changed inside or outside the business. Examples could include a new customer requirement, a change in services, a new process, staffing changes or a new supplier risk. Avoid vague wording such as “no changes noted” unless that is actually true and was reviewed.
A practical entry might say that a new customer now requires faster response times, so the service objective will be reviewed. That turns context review into a decision, not just a checkbox.
Performance and objectives
For performance and objectives, reference the measures the business already uses. These might include customer complaints, delivery performance, service response, rework, audit completion, training or process KPIs. The minutes should state whether objectives are being met and what action is needed where they are not.
A useful decision might be: “Customer complaints remain within target, but two late responses related to holiday cover. Service Manager to update the cover rota before the next peak leave period.” That wording is plain, auditable and action-based.
Audit results
Summarise internal audit results without copying the full audit report. Include whether audits were completed, what themes were found, and whether any nonconformities or improvement actions need management attention. If you use an internal audit checklist during implementation or preparation, the management review should consider the important findings rather than repeat every line.
Nonconformities and corrective actions
This section should link to the corrective action process. Record the number and type of open corrective actions, whether root causes are being addressed, and whether anything needs escalation. A strong entry explains what decision management made, such as assigning resources, changing a procedure or accepting that an action is complete because evidence has been reviewed.
Resources and competence
For SMEs, resources and competence are often practical: training gaps, workload, equipment, software, vehicles, subcontractors, or time to maintain the QMS. The minutes should capture whether resources are adequate and what will change if they are not.
Risks, opportunities and improvement
This is the place to pull together recurring issues. If audit results, customer feedback and corrective actions all point to the same process weakness, record it as an improvement priority. The decision should name an owner and a due date.
Actions and outputs: the part auditors will follow
The Actions and Outputs table is where the meeting becomes evidence. The form uses these columns: Action / decision, Owner, Due date, Status and Evidence. Every significant decision should land here, especially where the discussion creates a task.
| Action / decision | Owner | Due date | Status | Evidence |
|---|---|---|---|---|
| Update supplier scorecard and file the reviewed copy | Operations Manager | 30 September 2026 | Open | Updated supplier scorecard |
| Add a holiday-cover check to service planning | Service Manager | 10 October 2026 | Open | Revised rota and planning note |
| Review training needs for the new inspection process | Quality Coordinator | 20 October 2026 | Open | Training matrix update |
The “Evidence” column is especially useful. It reminds the owner what record will prove the action was completed. At the next review, previous actions can be checked against this table.
Common mistakes to avoid
Writing “discussed” without recording the decision.
Listing actions without owners or due dates.
Treating management review as a once-a-year paperwork exercise instead of a business review.
Copying clause wording into the minutes instead of using plain business language.
Forgetting to close the loop on previous actions.
Recording “suitable” without noting any evidence reviewed.
